- REQUIRED DOCUMENTS include the Florida Certificate of Immunization (Form #DH680), the School Entry Health Exam (Form #DH3040), your child's birth certificate, social security card, picture of child, family picture, parents' driver licenses or passports, and social security cards and/or visas with expiration dates are all needed for enrollment. All documents are needed at least two (2) business days prior to your child's start date. If your child is entering an Elementary class (1st-5th Grade) then a copy of your child's last report card showing completion of the previous grade level and a letter from your child's school is needed.

Great school! We are happy and look forward to seeing our child grow in this
school. The staff is warm and welcoming, they are bilingual and have a great curriculum and always clean. Aventura Learning Center is our #1 choice.
